This unit (CHCCCS001) describes the skills and knowledge required to determine and address specific issues faced by people who have a chronic disease, in the context of contributing to a holistic approach to support. The unit applies to workers providing services to people with chronic disease in diverse community services and health contexts. The skills in this unit must be applied in accordance with Commonwealth and State/Territory legislation, Australian/New Zealand standards and industry codes of practice.

Assessment
Assessment tasks will be designed to reinforce and extend knowledge and skill competence within set and controlled parameters in accordance with each unit's learning outcomes and performance criteria requirements, including the setting of work based practical application tasks designed to provide evidence of competence outcomes, within periodic and scheduled timelines.
Students will be expected to demonstrate the following required skills:
- determined and addressed the needs of at least 3 people presenting with different types of chronic disease and lifestyle situations.Students will also be expected to demonstrate the following knowledge:
- broad context for chronic disease;
- strategies to address and manage chronic disease in the relevant work context;
- ways to communicate appropriately and effectively with a person with a chronic disease, and;
- roles and responsibilities of different people in chronic disease management and their contribution to coordinated service delivery.
